Summary:Provides skilled occupational therapy in accordance with the physician's plan of care.

Expectations & Expected to be able to follow geographical directions.

* Expected to be able to provide transportation for self from house to house.

* Expected to have basic mathematical skills.

* Expected to attain basic knowledge of Horizon Homecare system.

* Expected to demonstrate a good working knowledge of CMS/AHCA/OSHA/The Joint Commission regulations for patient care and documentation.

* Expected to evaluate, and regularly reevaluate, the rehabilitation needs of the patient.

* Expected to initiate diagnostic, preventative and rehabilitative procedures as appropriate to the patient's condition.

* Expected to demonstrate and teach alternate techniques to complete activities of daily living, proper transfers and positioning.

* Expected to assist the physician in the evaluation and assessment of the patient's level of functioning.

* Expected to recommend solutions for complex patient care.

* Expected to develop, prepare and maintain individualized patient care, initial evaluations, progress notes, care plans, discharge summaries, and hand off communication document with accuracy, timeliness and according to policies.

* Expected to perform home aide supervisory visits when only skilled discipline on case or to educate caregivers/aides in home safety precautions or home exercise programs.

* Expected to participate in PI processes.

* Expected to apply diagnostic and prognostic procedures and tests to assist in the development and revision of a patient's plan of care.

* Expected to utilize agency educational material as appropriate.

* Expected to treat the patient through the use of therapeutic activities designed to restore function and self care activities for the purpose of improving function.

* Expected to recognize situations that require notification of supervisor.

* Expected to recognize the influence of beliefs and cultures on behaviors and accept strengths and limitations in others.

* Expected to exhibit adaptability, flexibility, self control and maturity in work and behavior.

* Expected to exhibit calm responsiveness to emergency or stressful situations.

* Expected to assist with the coordination of the total plan of care and maintain continuity of patient care by conferencing with other health professionals assigned to the same patients.

* Expected initiate and participate in care conferences for complex and/or multidisciplinary patients when required or helpful to ensure good coordination of care.

* Expected to communicate the plan of care changes to the scheduler as often as necessary.

* Expected to observe, record, and report the patient's reaction to treatment and any change in the patient's condition to the physician and/or nursing care coordinator and other staff involved.

* Expected to instruct and advise patient, family and other health team personnel in the exercise program designed to increase the patient's strength and control.

* Expected to cooperate with appropriate staff to provide staff education when requested and arranged by the nursing care coordinator.

* Expected to communicate verbally and in legible writing in the English language in a manner that can be understood.

Minimum Education and/or Experience:* Graduate of 4-year college Occupational Therapy Program* One year of experienceLicensure/Regulation/Certification:* Licensed in the State of Florida as an Occupational Therapist* BLS certification from American Heart Association* Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites.

* The person in this will work in a smoke-free location, and is expected to adhere to all smoking restrictions.

* In the spirit of wellness for both our employees and patients, Boca Raton Regional Hospital has instituted a mandatory influenza prevention vaccination program.

Current employees must receive the influenza prevention vaccination free of charge during the recognized flu season.

As a condition of employment, new hires will be required to comply with all program rules.

This is a mandatory program with the exception of documented medical or religious reasons for not receiving the vaccine.
