Practice Details: Facility 1: Shift Coverage: Double with 38 hours of MLP coverage.
Shift Times: Monday Friday: 6a-5p, 8a-7p, 2p-11p, 6p-5a.
Saturday/Sunday: 7a-7p, 7p-7a, 11a-11p, 5p-4a.
Patient Age Range: All.
Facility 2: Shift Coverage: Single coverage with 24 hours of MLP Coverage.
Shift Times: 6a-6p; 6p-6a.
