SummaryThe Director of Care Coordination is responsible for the planning, operations, and management of the Care Coordination department.

In this role, the Director has overall responsibility for the continuum of care services provided by the hospital.

The Director supervises a staff of RN Case Managers, Discharge Planners, Medical Social Workers, and other support staff.

The Director will be expected to work collaboratively with the organization's leadership, nursing directors, physicians, and Quality Management department.

In this role, the Director will actively participate in all organization-wide initiatives including but not limited to patient satisfaction initiatives, organizational programs, accreditation and other reviews, and the development of all applicable policies and procedures.

The Director is also responsible for the efficient utilization of resources within the department while promoting collaboration, quality care and customer service excellence.

Minimum Education and/or Experience Required:* Bachelor's Degree required; Master's degree preferred* Five Years of progressive Case management leadership management experience* Five to ten years of Care Coordination/Case Management experience* ACM/CCM certification preferred* Florida Registered Nurse License in good standing* Knowledge of case management principles, care transitions, readmission healthcare management, utilization review , regulatory compliance and reimbursement such as Value Based purchasing Excellent critical-thinking, problem-solving and analytical skills* Ability to lead and motivate others* Strong self-initiative and interpersonal communication skillsDepartment Specific Skills: Care Coordination* Excellent knowledge and experience in Utilization and Case Management* Supports and participates in Quality Management to assess performance improvement, and to address priorities for improved quality of care and safety, including supporting the use of corrective and preventative actions.

* Participates in the establishment and maintenance of effective working relationships with community service organizations and public agencies to insure the Case Managers and other applicable department staff members offer appropriate resources to patients and families.

* Ensures department knowledge of compliance with all applicable local, state, and federal policies, laws and regulations pertinent to care coordination, utilization management, discharge planning and provision of social services, including but not limited to The Joint Commission, CMS, State of Florida, and AHCA.

* Responsible for implementation and compliance of the Hospital utilization management program.

Reviews utilization patterns, identifies and reports trends.

Develops strategies to effectively optimize the hospital's resources.

* Reports and participates in All Hospital wide committees; such as Utilization Management, Transitions of care, Revenue Cycle, Patient Safety, Readmissions, HCHAPs and Value based Initiatives with the Hospital Quality Committees and others as needed* Coordinates Post-acute provider communications for seamless hand off of patients including integration with the hospital's Boca Raton Home Health AgencyLeadership* Plan, organize and direct the provision of services within the assigned department(s) to assure compliance with the organization's policies and procedures as well as Joint Commission Accreditation and appropriate regulatory agencies.

* Directs process of interviewing, hiring and assuring evaluation and competency of all personnel for areas of responsibility.

* Demonstrates efficient organizational and time management skills through consistent patterns of work behavior and positive representation.

* Helps provide educational development of staff in all aspects of Care Coordination, Utilization Management and Fiscal Responsibility (Reimbursement Methodologies).

* Provide leadership and guidance in the development, implementation, and evaluation of all Case management services in the department inclusive to providing education for all staff pursuant to federal and state regulations* Provide overall planning, control, organization, and monitoring of services and the employees within these areas.

* Plan, organize and direct the provision of services within the assigned department(s) to assure compliance with the organization's policies and procedures as well as The Joint Commission and appropriate regulatory agencies.

Performance Improvement* Responsible for operational excellence of all assigned activities to ensure the cost-effectiveness and efficient delivery of quality services and/or outcomes required to meet the expectations of those utilizing or impacted by the department.

* Develop and implement a plan for an effective, interdisciplinary and ongoing program for quality assessment and performance improvement in the areas of responsibility.

* Pursue, in a creative manner, continuous quality improvements, which increase customer satisfaction and organizational financial performance.

* Adapt procedures and utilize available resources to maintain quality services and to meet changing requirements.

* Follow all facility, organization and department precaution s and procedures in the performance of all to ensure a safe work environment for self and others.

Administrative Support* Assist Administration in analyzing, monitoring and controlling the budgets and operation of assigned areas to ensure cost effective and efficient delivery of quality patient care.

* Develop, implement, and manage the departmental budgets.

Direct and analyze staffing schedules to ensure that they are consistent with budgetary constraints and meet the department standards and guidelines.

* Communicate with Administration in a timely and routine manner on issues, potential problems, activities, and projects.

* Participate in the development of hospital-wide programs, policies, and procedures.

* Attend assigned and required hospital and organizational supported meetings.

* Support an environment that promotes departmental and organizational objectives by providing cooperative, courteous and professional service to all customers.

* Achieve high visibility and availability to employees, patients and families, physicians, and Administration to promote early identification and resolution of problems.

* Operate as a team with co-workers by approaching relationships in a positive manner.

* Respect confidentiality of patient, staff and privileged hospital information.

* Expected to serve as a role model in the participation and support of the Hospital Mission, Vision and Values.

* Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites.

* The person in this will work in a smoke-free location, and is expected to adhere to all smoking restrictions.

* In the spirit of wellness for both our employees and patients, Boca Raton Regional Hospital has instituted a mandatory influenza prevention vaccination program.

Current employees must receive the influenza prevention vaccination free of charge during the recognized flu season.

As a condition of employment, new hires will be required to comply with all program rules.

This is a mandatory program with the exception of documented medical or religious reason for not receiving the vaccine.
